Restore deleted forms

You can restore deleted rows of a repeating form, deleted rows from the repeating section of a two-section form, as well as instances of two-section forms and lab forms.

If a subject's status is Completed, Screen Failed, or Withdrawn, you can still restore deleted form records for that subject.

To delete or restore forms, you only need to be assigned the existing permissions: View Sites, View Form Data for Subjects, and Edit Data for Subjects.

Consider the following:
  • Before you restore deleted forms, read more about the impact on collected data when you perform these tasks. See About deleting, restoring, and viewing deleted form records.
  • You cannot restore cleared data.
  • You cannot clear data from deleted form records or raise queries against them. As soon as you restore a form record, you can clear its data or raise queries against any questions.
  • When viewing deleted forms that were used in multiple visits, you’ll only see them within the context of individual visits - not a comprehensive list of all deleted repeating forms across the entire study.
  • Subject or visit-level filters do not apply to the deleted form records shown on the Deleted Forms or Deleted History pages.
  • With the appropriate permissions, when looking at deleted form records, you can view hidden and classified data - and, if permitted, restore it - on the Deleted Forms or Deleted History pages.
  1. On the Home page, determine where to work. For more information, see Access study modes and pages.
  2. Along the top, make sure Subjects is selected.
  3. Filter your view based on your desired criteria. For more information, see Filter subjects in a study.
  4. Locate the subject and click the visit that you want to access.
  5. In the visit, locate the form whose records or instances you want to restore.
  6. Next to the form's title, do one of the following:

    The Deleted History or Deleted Forms links only appear when there are deleted form records for the form.
    • If you want to restore a two-section form instance, select Deleted History.
    • If you want to restore a repeating form record, select Deleted Forms.
  7. On the page where the deleted form records are displayed, do one of the following:

    Tip:

    To better determine which repeating form row to restore, you can also select the row number in the table. This opens the deleted repeating form in a new dialog. In there, you can look at the questions in more detail and browse the Answer & Visit History sidebar.
    • If you want to restore a single repeating form row, select Restore on the Action column.
    • If you want to restore multiple repeating form rows, select the checkbox next to each form row that you want to restore, and select Restore All Selected.
    • If you want to restore one or multiple two-section form instances, select the checkbox next to each instance that you want to restore, and select Restore All Selected.

      If you restore an instance of a two-section form, all repeating section rows are restored, even if they were deleted prior to the whole two-section form instance being deleted.

  8. On the Restore Selected Forms? dialog, select a reason for this change, and select Restore or Restore All Selected.
    The dialog also informs you that you will have to link any forms previously linked to or from this form record again.
    A message is displayed that confirms the restoration of the form record.

For each repeating form record or two-section form instance that you restore, the Answer & Visit History sidebar displays a Form Restored action, along with the reason specified for restoring the form, the date of the action, and the user who performed this action.
