Create a question to capture a verbatim term

Create a question to capture a verbatim term, such as a medication like Ibuprofen, an adverse event like a headache, or a medical procedure like an Appendectomy.

  1. Open the form and select Add, and then select the Text option.
  2. Enter the question’s title, for example, Medication taken.
  3. On the Details sidebar, set the Character Limit to 200, and if applicable, define other settings for the question.
  4. Expand the Advanced sidebar, select Add Property, and then select Coding Question.
    1. Select the appropriate Dictionary.
    2. Select Verbatim Term from Coding Item Type.
    3. Select the appropriate Tag for Central Coding. Tags are used in the assignment rule configuration in Oracle Central Coding to organize verbatim terms into groups.

      Tag Select for
      AE The verbatim term question on your Adverse Event form.
      DISEASE The verbatim term question on your Medical History form.
      NT-DISEASE The verbatim term question on your Non-Target Disease form.
      MEDPROD The verbatim term question on your Concomitant Medications form.
      LABDATA This tag can be used as needed to further distinguish between verbatim terms in Oracle Central Coding.
  5. Select Save.
