Create a file upload question

Create a file upload question to let site staff or other study team members upload relevant documents in forms.

Note:

Consider the following:
  • You can include a File Upload question in most form types, except for a Drug Reconciliation form. You can also include a File Upload question in a question group.
  • You can include and test a File Upload question in a library study, as well. For more information on library studies, see About library studies.
  • You cannot configure a File Upload question to be a coding question and you cannot specify EHR (Electronic Health Records) mappings.
  • You cannot upload documents in Preview mode, while designing a study.
We recommend you always configure a File Upload question as editable, as read-only does not allow any user to upload a document. If you need to apply any restrictions on the File Upload question, use data classifications to ensure that only certain users can upload documents.

To add a File Upload question, you must first create a form. For step-by-step instructions on how to create various types of forms, see Forms.

To create a File Upload question:

  1. On the form, select Add, and then select File Upload.
  2. On the right, expand the Details sidebar and, if needed, modify the reference code for the question.
    A reference code is a one-word abbreviation for the question.
  3. In the Question Hint field, provide site users with guidelines for data entry, if necessary.

  4. If this question is required to be answered for a specific event (whether it is visit completion or screening, for example), make sure you select the appropriate checkbox.   5. If you want to classify the File Upload question so that only specific users can upload and view the uploaded document, turn on the Hidden toggle Hidden and specify the exact data classifications this question should have.   6. To configure source data verification settings at a question level, see Configure source data verification settings for a question.
  7. Expand the Advanced pane and make sure the SAS properties are correctly defined for both the SAS Variable and SAS Label fields.
  8. To create an Auto-lock validation rule, see Define an Auto-lock rule.
    Auto-lock rule is the only available predefined rule option that you can configure for a File Upload question.
  9. To save the form, in the lower right of the form, select Save or Save and Close.

    Tip:

    If the Save button is disabled, make sure there are no blank questions on the form.
