Most all metric extension operations can be carried out from the Metric Extension home page. If you need to perform operations on published extensions outside of the UI, Enterprise Manger also provides EM CLI verbs to handle such operations as importing/exporting metric extensions to archive files and migrating legacy user-defined metrics to metric extensions.
