Upload or delete documents from forms

You can upload or delete documents in forms to provide additional information for subjects in a study.

You can only upload a single file for each question in a form. During the uploading process, you can view the file's upload progress, and you can cancel the upload or refresh your page, but you cannot modify forms or subjects during the upload.

When you upload a document in your form, you must consider the following requirements:
  • For your security and to protect the system, uploaded file names shouldn't be longer than 150 characters and cannot include:
    • Special characters that are commonly used in programming or scripting, such as: semicolons
      ;
      or comment symbols
      - -, #, /* … */
    • Words that are used in database commands, such as: INSERT or UNION.
  • A file's maximum size can only be 10 MB.
  • The following document types are supported: DOC, DOCX, XLS, XLSX, PPT, PPTX, PDF, JPEG, JPG, PNG, GIF, and TIFF.
  • To upload a new version of the same document, you must first delete the current version uploaded to the form.

Note:

When an upload question is frozen or locked, you cannot upload or delete a document for that question. Contact your sponsor team to get that question unfrozen or unlocked. However, you can upload or delete a document for an upload question that is signed or verified. After the document is successfully uploaded or deleted, the question becomes unsigned or unverified.
  1. On the Home page, determine where to work. For more information, see Access study modes and pages.
  2. Along the top, make sure Subjects is selected.
  3. Filter your view based on several criteria. For more information, see Filter subjects in a study.
  4. Locate a subject on the subjects list, and select the desired visit.
  5. Locate the upload question on your form and select Upload, then do one of the following:

    Tip:

    If you apply a data flag to an upload question, you cannot upload a document anymore. To upload a document, remove the data flag, and specify a reason for this change. After this, you can upload a document again without specifying another reason for this update.
    • On a one-section form (flat), whether individual or included in a two-section form, look for the Upload button, which is displayed directly on the form.
    • On a repeating form or the repeating section of a two-section form, look for the Upload button inside the repeating form table. You can also select the row number to view all repeating form instance questions (including the Upload button) in a separate dialog.
  6. Select the document that you want to upload, then select Open.
    A dialog appears where you can view the file upload progress. If needed, select Cancel to stop the upload process.
    • If successful, a pop-up message is displayed indicating that your file was uploaded successfully.
    • If you cancel the upload (manually or through refreshing the browser's page), another pop-up message is displayed indicating the cancellation.
    • If the upload fails for technical reasons, an appropriate pop-up message is displayed with the cause of this error.
  7. To view an uploaded document, select the file link created after the upload is complete. For a repeating form, select the row number to view the file link.
    Depending on your browser's settings, the file may open in a new tab in the browser (for example, an image or a PDF document) or the document is downloaded to your computer (for example, a Word document).
  8. To delete an uploaded document, select the icon (Delete) next to the file link. On the Delete the File dialog, select a reason for this change from the Reason for Change drop-down, then select Delete.
  9. To view the history of an uploaded file, expand the Answer & Visit History sidebar.

    Tip:

    Due to the unique nature of this question type, to view the history of an upload question, place your cursor in the question above or to the left of the upload question and use the Tab key until the question is highlighted. You can also place the cursor in the question to the right or below and use Shift+Tab on your keyboard. After that, expand any sidebar on the right of the screen (the Question Hint sidebar, the Answer & Visit History sidebar, and others).
