| Fields | Description |

| Site ID | A site's ID as entered by the site manager when they created the site. |
| Site Name | A site's name as entered by the site manager when they created the site. |
| Kit Type | Indicates the type of kits as All, Serialized, or Non-Serialized. |
| Kit Shipping Address | Address where the kit will ship to.
Note: Locally sourced kits display as Locally Sourced. |
| Last Updated By | User name of the user who last updated a site's kit shipping details. |
| Last Updated Date and Time | UTC date and time of the latest update. |
| Operation | Indicates the type of operation performed: Added, Removed, or Updated. |
