Lab Form Indicates it is a lab form.
Calculate doses in a kit type Indicates that a question in the form is used for calculating doses in a kit type.
Form copied from another study

Indicates the form has been copied from another study.

You can hover over the icon to reveal the study the form was copied from and when it was last modified.
Form is copied with some or no rules Indicates the form has been copied from another study with some or no rules.
Rollover study question Indicates a question in the form has been used in a rollover study.
Question used for randomization Indicates a question in the form is used to stratify subjects at randomization.
Assign subjects to a branch Indicates that a question in the form is used to assign subjects to a branch.
