Cause
The instance terminated because the storage was inaccessible from the site that sent the disk OFFLINE request.
Action
Run the command 'crsctl query cluster sites <site_name>' to retrieve the status of the indicated site. Address the storage accessibility issues if the indicated site is in a QUARANTINED state. Run the command 'crsctl modify cluster site <site_name> -s rejuvenate' to set the status to ONLINE after the storage access has been restored.
