Cause
SCNs for replay are not valid on target database. The system change numbers (SCN) recorded during the original execution were not valid on the new database after failover. This might happen if the new database was opened with RESETLOGS option, or if the new database was set up to allow significant lag in replication. The failover could not continue.
Action
Check the SCNs on target and source, and whether RESETLOGS was used when opening the target database.
